【本報綜合報道】現代人飲食習慣愈來愈不健康,英國劍橋大學聯同美國兩個學術機構最新研究發現,十個常見的飲食壞習慣容易演變成中風、心臟病及二型糖尿病,並因而導致死亡。該項研究分析了二○一二年美國約七十萬名因上述疾病死亡者的生前健康資料,發現當中四成五與不良飲食習慣有關。</div>

位列「死亡榜首」的飲食壞習慣是攝取太多鹽分,佔死者比例的9.5%。排第二的壞習慣是進食不足堅果類食物,佔8.5%;其餘八個壞習慣包括食太多加工肉類、攝取太少魚類含有的奧米加-3、蔬菜不足、水果不足、過分嗜甜飲、進食不足全穀類食物、多元不飽和脂肪攝取過低、食太多紅肉。學者指出,單單改善飲食習慣已可減低三至七成患心血管疾病的風險,呼籲市民要飲食健康。</div>

腎結石是尿液中的礦物質結晶沉積在腎臟裏,當結晶體愈積愈大時,便會堵住輸尿管。泌尿外科專科醫生黃國田指腎結石成分複雜,但有80%屬草酸鈣,是一種有機化合物,存在於環境及膳食中,當人體從食物中攝取到草酸及鈣質後,會結合為不可溶解的草酸鈣,因無法被人體吸收,會經泌尿系統排出體外。</div>

黃國田表示,「菠菜和豆腐不能一起吃」的說法,已被2002年刊登於《新英倫醫學雜誌》的一篇論文打破,因研究證實人體需要攝取相同比例的草酸和鈣質,才能互相溶解,「一旦偏向攝取草酸或鈣質,都一樣會增加患腎結石的機會。」他續指,過量攝取鈣片,才是腎結石元兇。他解釋,當鈣質的濃度或攝取分量過盛,尿液中的結晶體會隨之積累,長時間累積會形成腎結石。</div>

早期症狀難察</div>

早期腎結石因無症狀較難發現,黃指腎結石有先天家族遺傳因素,以及後天飲食習慣影響,包括足夠水份及均衡飲食,「礦物質是腎結石來源,一旦超過能溶解的能力就會生石。」因此必須多飲水增加排尿,每次至少要排走2.5至3公升的尿液,即每日要攝取相同分量水份,相等於8杯水、果汁、清茶或湯等,「其中至少有4杯要係水」。他表示,有患者經過數十年後,才發現有腎結石,當腎石在堵塞腎狀內部,會出現劇烈腰痛,甚至造成泌尿系統磨損,排出血尿,一旦被細菌感染,併發症包括發燒、泌尿系統含膿等,隨時威脅生命。</div>

如何才能預防生腎石?聖雅各福群會社區營養服務及教育中心註冊營養師關建慧稱,成人每人每日建議攝取1,000至1,200毫克鈣質,「大劑量吸收鈣片,會增加患腎結石」,以一片鈣片為例,已達300至1,000多毫克不等,即吃兩片已有機會超標。</div>

<abstracttext>Eggs are a source of <span class="highlight">cholesterol</span> and choline and may impact plasma lipids and trimethylamine-N-oxide (TMAO) concentrations, which are biomarkers for cardiovascular disease (CVD) risk. Therefore, the effects of increasing egg intake (0, 1, 2, and 3 eggs/day) on these and other CVD risk biomarkers were evaluated in a young, healthy population. Thirty-eight subjects [19 men/19 women, 24.1 ± 2.2 years, body mass index (BMI) 24.3 ± 2.5 kg/m<span style="font-size: 0.8461em; line-height: 1.6363em; position: relative; top: -0.5em; vertical-align: baseline;">2</span>] participated in this 14-week crossover intervention. Participants underwent a 2-week washout with no egg consumption, followed by intake of 1, 2, and 3 eggs/day for 4 weeks each. Anthropometric data, blood pressure (BP), <span class="highlight">dietary</span> records, and plasma biomarkers (lipids, glucose, choline, and TMAO) were measured during each intervention phase. BMI, waist circumference, systolic BP, plasma glucose, and plasma triacylglycerol did not change throughout the intervention. Diastolic BP decreased with egg intake (P < 0.05). Compared to 0 eggs/day, intake of 1 egg/day increased HDL <span class="highlight">cholesterol</span> (HDL-c) (P < 0.05), and decreased LDL <span class="highlight">cholesterol</span> (LDL-c) (P < 0.05) and the LDL-c/HDL-c ratio (P < 0.01). With intake of 2-3 eggs/day, these changes were maintained. Plasma choline increased dose-dependently with egg intake (P < 0.0001) while fasting plasma TMAO was unchanged. These results indicate that in a healthy population, consuming up to 3 eggs/day results in an overall beneficial effect on biomarkers associated with CVD risk, as documented by increased HDL-c, a reduced LDL-c/HDL-c ratio, and increased plasma choline in combination with no change in plasma LDL-c or TMAO concentrations.</abstracttext></div>

研究人員用數學模型,來模擬動物是如何對不同的食物供應作出反應,結果發現,現時的人類與1.2萬年前的人類差不多,面對食物供應不穩定的情況,就會主動吸收多一點,就像雀鳥要儲存能量度過寒冬一樣。所以「搖搖式」減肥的人,在偶爾不節食的階段,身體會吸收更多營養,容易令體重飈升。</div>

香港營養學會會長張智良指,港人減肥逾半會選擇節食,卻不配合運動,若加上「搖搖式」減肥,則更易愈減愈肥。他解釋,肌肉的多少控制着人體的新陳代謝率,而節食減肥通常會減少肌肉;透過節食雖可減少攝取卡路里,在初期易見效,但隨着肌肉愈來愈少,身體沒有足夠的肌肉來消耗攝取的能量,減肥效果就會愈來愈差。若間中「破戒」吃多一點,身體更難消耗攝取的熱量,令體重升得更快。</div>

張又指,近期多節慶及派對,節食者容易「破戒」,節日後是體重反彈的「高危期」,「不少人的節食計劃太進取,遏抑了慾望,堅持1個月就難以控制,加上大時大節很多聚會,節食者面對誘惑,很難堅持。」曾有中四女生,原本體重約70公斤,選擇不吃早餐或午餐減肥,一個月減掉2公斤,但聖誕、元旦及農曆新年期間經常出席派對,事後體重竟反彈增加了5公斤。張提醒,只靠節食減肥容易愈減愈肥,應配合適量運動才更有效。</div>

家庭醫生林永和表示,生病時對抗病菌靠本身抵抗力,即使食維他命C也太遲,不會有即時功效,形容「考試前臨急抱佛腳溫書,根本就趕唔切」,只是心靈「安慰劑」,感覺像「自己做緊嘢」。「家營營養中心」營養師陳筠霖指,醫學上未有足夠證據證明維他命C有助預防流感或助感冒痊癒,但攝取足夠維他命C有助製造對抗病菌的淋巴細胞,增強免疫力。</div>

對如何預防感冒,林永和認為,注意個人衞生更重要,「流感是經飛沫傳播,故摸完攔杆及枱面,進食前要洗手,減低將病菌帶入口腔風險,也避免常捽眼及捽鼻。」生病時多休息,讓身體有能量對抗病菌。他坦言:「病唔病有很多因素」,包括平日休息及運動是否足夠,飲食是否均衡等,而維他命C的影響極細。</div>

一般而言,成年人建議每日攝取維他命C為90毫克,不應超過2,000毫克。林永和指,對本身缺乏維他命C人士,食維他命C丸或飲沖劑有其作用,但從飲食上已攝取足夠維他命C的人,再補充則變得多餘,「維他命C是水溶性,過多身體也不會吸收,經尿液排出體外。」</div>

陳筠霖說,一般維他命C沖劑或丸含1,000毫克維他命C,遠超於建議攝取量。日常維他命C飲品、水果及蔬菜皆有一定含量,加起來很易超過建議上限。而部分人服用這類產品時,未必清楚閱讀食用指示或憑記憶判斷分量,易超出上限而不自知。本港曾有男子因每日服用4,000毫克維他命C導致腎衰歇,需入院長期洗腎。</div>

陳筠霖表示,番石榴的維他命C相比柑橘類水果高出2至3倍,蔬菜中以紅燈籠椒含維他命C最多。以半杯番石榴為例,含190毫克維他命C,但一個橙及半杯士多啤梨分別只是69.7及48.8毫克。</div>

2003年起食素的馬女士指,從前到茶樓會吃馬拉糕及蓮蓉包等,以為吃素較健康,豈料經測試後發現壞膽固醇嚴重超標,反映「吃素非無敵」,要視乎食物類別。57歲的簡先生則是龍虎舞獅,今年兩度中風,其後發現因日常進食太多含糖量高的食物、吸煙及飲酒所致。</div>

腦神經科專科醫生盧文偉指,膽固醇超標與常食快餐、高脂高糖食物或煙酒有關。盧表示,可透過多吃水溶性膳食纖維及每天吸收2克飽和植物固醇達到降膽固醇功效,花生、紅蘿蔔、蘋果和全麥麵包都含有飽和植物固醇。港大心臟血管研究所主任張文勇指,每日吸收2克飽和植物固醇有效減少10%至15%壞膽固醇,效果較進食雙倍降膽固醇藥更佳。</div>
